Further to Mayoral Question 2024/0778, will you provide me with a copy of the Notice of Contravention TfL received from the HSE for the death of a 74-year old female pedestrian on 15 December 2023 after being hit by a Route 212 Go Ahead London bus working under contract to TfL? This Notice of Contravention is mentioned in the July 2024 TfL Commissioner's report and also in a 22 August My London report.

Given your ambitious "Vision Zero" goals, can you explain why the latest (July 2024) TfL Commissioner's Report to the TfL Board states, "We completed improvements at Victoria bus station in December 2023..." without mentioning the fact that a pedestrian was killed by a Route 14 bus operated by RATPDev for TfL in January 2024?

Can you provide me with some further detail about the EQM process, including a description of how the EQM process's success or failure is monitored?

Please note that I've been informed by bus drivers that TfL's Tripartite Toilet Working Group hasn't met for 18 months.

Her immediate predecessor at TfL was just appointed to the Board of Directors of TfL's largest Contractor, Go Ahead.
