"City Sightseeing and City Tour" Bus Crash - September 13 - Waterloo Station

  • Reference: 2024/3436
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
On 13 September a pedestrian was seriously injured after being struck by, reportedly a "City Sightseeing and City Tour" bus at Waterloo Station. Does this bus tour operator have any links to any bus contractor providing public bus services to TfL? If so, will TfL be reporting it under that Bus Contractors' Quarterly Bus Safety Performance Data for Q3 2024?

Vision Zero: Misleading about Bus Safety Performance by the TfL Commissioner

  • Reference: 2024/3435
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
Further to the issue raised in Mayoral Question 2024/2895, on 4 September, I questioned the TfL Commissioner Andy Lord about bus safety at Victoria Station and on the network that the record shows he responded "the overall number of fatalities on the bus network has continued to reduce year-on-year”. Since the total number of deaths from 2024's preventable bus safety incidents appear to have exceeded 2023's total by August, do you believe the TfL Commissioner has misled the London Assembly about TfL's declining bus safety performance?

Bus Drivers Dangerously Overheating: Success of EQM Process

  • Reference: 2024/3434
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
How do you reconcile your responses to Mayoral Questions 2024/2893 and 2024/2238 with the horrific conditions suffered by bus drivers revealed in the 21 September My London investigation entitled "Whistleblower London bus drivers ‘exhausted’ and ‘use faulty vehicles’ in ‘dangerous’ conditions”?

Working Groups of the Tripartite Meetings

  • Reference: 2024/3433
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
In your response to Mayoral Question 2024/2892, you stated "There is not a specific toilet working group." What 'specific' working groups are there and when were they created?

Vision Zero: TfL Monitoring of Unsafe Bus Driver Working Conditions

  • Reference: 2024/3432
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
How do you reconcile your responses to Mayoral Questions 2024/2238 and 2024/2892 with the horrific conditions suffered by bus drivers revealed in the 21 September My London report entitled "Whistleblower London bus drivers ‘exhausted’ and ‘use faulty vehicles’ in ‘dangerous’ conditions"?

Footwell Cameras and London's Bus Vehicle Specification

  • Reference: 2024/3431
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
Following up Mayoral Question 2024/2891, please will you provide me with the exact date TfL decided footwell cameras were to be included in London's ‘Bus Vehicle Specification’ and any documentation (e.g. Decision Memorandum) that supported this change?

Safety Priorities

  • Reference: 2024/3430
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
Following up your response to Mayoral Question 2024/2890, how do you reconcile the fact that more buses have had footwell cameras installed – ostensibly to assist TfL in establishing the frequency of pedal confusion events which, in Mayoral Question 2024/2889 you state are "rare" but "under-reported" – than those that have been equipped with ISA technology—ostensibly to assist TfL in controlling 12 million (my estimate based on bus operator statements) of ‘Bus Driver Speeding Events’ for which TfL does have data from its speed compliance tool found in the iBus System on every London bus?

Vision Zero: Under-Reporting of Systemic Safety Failings and Bus Crashes

  • Reference: 2024/3429
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
In response to Mayoral Question 2024/2889, you admitted that TfL suspects bus drivers may “be under-reporting" pedal confusion incidents. Given the "culture of fear" described by bus drivers revealed in the 21 September My London report ("Whistleblower London bus drivers ‘exhausted’ and ‘use faulty vehicles’ in ‘dangerous’ conditions"), might that explain why (a) "In reports since April 2016, the need to urgently use a toilet is only mentioned by bus drivers in a very small number of safety incident reports" (cf. Mayoral Question 2022/3474), and (b) why from 2018 to 2022 TfL only records 5 incidents where bus drivers crashed...

Under-reporting' of Pedal Confusion Events

  • Reference: 2024/3428
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
In your response to Mayoral Question 2024/2889, you stated, "Pedal application error events are rare, but it is also likely that they are to some extent under-reported”. Please will you provide me with any data, analysis or independent analysis that support your belief that pedal confusion events are under-reported?

Revolving Door between TfL and its Bus Contractors

  • Reference: 2024/3427
  • Question by: Neil Garratt
  • Meeting date: 10 October 2024
Following up on your response to Mayoral Question 2024/2888, does TfL's 'robust process for assessing conflict of interest for all roles' include asking a) the current Director of Buses to recuse herself from any matters involving TransportUK, which currently provides nearly 10 percent of TfL's contracted Bus Fleet, b) the former Director of Buses to not be involved in any matters involving Go Ahead London, which currently provides over a quarter of TfL's contracted Bus Fleet, and c) the former MD for Surface Transport to not be involved in any matter involving RATP, which currently provides over 12 percent of...
