On Tuesday 13th May the destination of a morning rush hour Metropolitan line train was changed four times between Pinner and Finchley Road. Why did this happen and was correct protocol followed at all times?

On Thursday 8th May two trains were stuck for some time at the platform at Harrow-on-the Hill Underground Station as they had no drivers. Commuters reported that station staff appeared to be unsure what they should be doing. How did this situation occur and what should station staff have done?
