One of the key conclusions of the Rail Accident Investigation Board report about Sandilands was the absence of a ‘just culture’ at First Group Tram Operations Limited, which undermined safe working conditions and practices. Meanwhile, while your Chief Safety Officer regularly praises the ‘collaboration’ TfL has with bus operators in ‘safe places’ free from public scrutiny, your bus drivers regularly provide evidence to me that London’s bus operation’s culture is, for the lack of a better word, toxic. Do you think London’s bus operators, which TfL oversees, have created and maintained a ‘just culture’ for the bus workers?

A recent Financial Times article gave the gruesome details of a woman who lost her arm and leg after falling on the tracks at High Barnet Tube Station. The article explained that the unsafe conditions at High Barnet Tube are not unique and can be found elsewhere across the Tube network with old platforms. Can you provide me with the risk assessments for people falling on Tube tracks at High Barnet Station and others with a similar dangerous configuration for customers?
