Bus Collision Fatalities since the January 2009 RAIB Report on the September 2008 Fatal Croydon Tram Crash

Since the release of the January 2009 RAIB Report, 98 people have died as a result of collisions involving a TfL Bus. How many of these fatalities involved a passenger being ejected from the window of the top deck of a double decker Bus? During the same period, how many passengers have been injured (seriously or hospitalised) as a result of being ejected from the top window a double decker bus after a collision?

RAIB Recommendations to TfL Buses after the Fatal 2008 Croydon Tram Crash

The 2009 RAIB Report on the September 2008 Fatal Crash between a Croydon Tramlink Tram and a TfL Bus expressed specific concerns about the design of the front upper window of the double decker bus not affording passengers enough protection in the event of a crash and, in this particular incident, resulted in the victim being ejected through this window and dying. What actions did TfL take to improve the upper deck front window design of double decker buses as a result of the January 2009 RAIB Report?
