Thank you for choosing to take part in our survey.

This survey is for organisations. If you are an individual and would like to respond to the draft Health Inequalities Strategy, please visit our Talk London discussion pages.

The survey contains questions on each theme of the Draft Health Inequalities Strategy. Please feel free to only answer questions that are relevant to you or your organisation.

Please note, this form must be completed and submitted in one attempt, there will NOT be a chance to save your answers halfway through, but you can upload your response as a document if you prefer (in the further information section).

A series of questions on each area of the draft Mayor’s Health Inequalities Strategy:

  • Healthy Minds
  • Healthy Place
  • Healthy Communities
  • Healthy Habits

We recommend reading through all of the questions for each area of the strategy that you are interested in before beginning to answer them. 

Each group of questions will also include a link to take you to the relevant section of the draft Health Inequalities Strategy if you would like to refer back to this before answering.

In this secion you can upload your response as a document, or provide additional information to support your response if you prefer.

This section also includes the Integrated Impact Assessment for the draft Mayor’s Health Inequalities Strategy.

We will consider all the feedback received and will use it to prepare a report to the Mayor to inform the development of the final strategy.

We expect to publish the final version of the Health Inequalities Strategy in 2018.
