Why did TfL conduct so few audits of the fitness and competence of Tram Operations Limited's Tram drivers? How often does TfL intend to carry out such audits in the future?

Given what we now know about the likely causes of the Croydon Tram crash in November 2016 are you confident that such a statement by TfL is still applicable?

    According to TfL's own data over the 10-year period 1 April 2007 to 31 March 2017, TfL has recorded 249,835 collisions involving TfL buses, 161 which have resulted in fatalities. In how many of these collisions was driver fatigue recorded as a factor or contributing cause?
