Assuming this question relates to the relationship with bus operating companies, we can confirm that the reporting process requires that all safety-related incidents on the bus network are reported to TfL using a web-based system.

Bus operators are contractually required to conduct thorough investigations in the event of serious incidents. To support the sharing of lessons learnt, the conclusions of major incidents are provided to TfL. Where it is appropriate to do so, TfL conducts joint investigations with the bus operators involved.
